We are seeking professional and compassionate Qualified Professionals (QP)/Associate Professional (AP) for our growing Intensive In-home team. The QP/AP will be responsible for the development, implementation, monitoring, and revision of the Person-Centered Plan (PCP) in conjunction with the interdisciplinary healthcare team. This position works with the child/adolescent population. Providing Skill building, and psycho-education to promote behavioral modification.
Responsibilities:
- Supporting the consumer in the development of various skill-building activities such as daily and conflict resolution skills, socialization skills, appropriate behavior, and anger management skills.
- Performing case management functions of linking and arranging for services and referrals.
- Providing care coordination by working closely with other clinical/professional staff to maintain communication and provide feedback.
- Creating and implementing PCPs, expeditiously.
- Facilitating relationships and serving as a link between the company, parents/guardians, schools, and local agencies.
- Team Oriented.
